Beslisregel voor diep veneuze trombose (NHG)
De afzonderlijke anamnestische gegevens en de bevindingen bij het lichamelijk onderzoek hebben een beperkte voorspellende waarde voor het stellen van de diagnose DVT en het uitsluiten ervan.(bron) Door gebruik te maken van deze beslisregel waarmee een risicoscore wordt berekend, kan de huisarts onderscheid maken tussen een verhoogd of niet-verhoogd risico op aanwezigheid van DVT.

De beslisregel kan niet worden gebruikt bij zwangeren en vrouwen in de kraamperiode, bij klachten die langer dan dertig dagen bestaan en bij patiënten die anticoagulantia gebruiken.

  • Risicoscore beslisregel DVT ≤ 3 én D-dimeer negatief of normaal: DVT voldoende uitgesloten.
  • Risicoscore beslisregel DVT ≥ 4 óf D-dimeer positief of verhoogd: verhoogd risico op DVT. Vraag dezelfde dag een 2-puntscompressie-echografie aan.(bron)
