Context informatie:
De mate van sedatie bij een patiënt op de intensive care (IC) kan worden geobjectiveerd met de Richmond Agitatie en Sedatie schaal (RASS). Bij een score van -5 / -4 is er sprake van een delier. Bij een score van -3 tot -1 is er een vermoeden van een hypoactief delier. Bij positieve scores is er sprake van (vermoeden van) een hyperactief delier.

Instructies voor gebruik: 
Onderzoek ter beoordeling van de mate van sedatie verloopt in enkele stappen, te weten:

  • Stap 1. Observeer de patiënt. Indien de patiënt alert, rusteloos of geagiteerd is zonder stimulatie, worden deze gescoord tussen 0 en + 4.
  • Stap 2. Indien een patiënt niet spontaan alert is, spreek de patiënt aan met zijn naam en zeg dat hij zijn ogen open moet doen. Indien de patiënt reageert op aanspreken, ligt de score tussen de -1 en -3.
  • Stap 3. Als de patiënt niet reageert op verbale stimulatie, geef een fysieke stimulatie (bv. schudden aan schouders of pijnprikkel). Afhankelijk van de reactie kan de patiënt als -4 of -5 gescoord worden.
Gehanteerde definitiies:
  • Strijdlustig: oppositioneel/vijandig, gewelddadig, gevaar voor personeel.
  • Erg geagiteerd: trekt aan of verwijderd katheter(s) of tube(s), agressief. 
  • Geagiteerd: regelmatig niet doelgerichte bewegingen, afwerende reacties. 
  • Onrustig: angstig maar bewegelijkheid is niet agressief krachtig. 
  • Slaperig: niet volledig alert, maar is in staat wakker te blijven (ogen open) bij stemgeluid (>10 seconde). 
  • Lichte sedatie: kort wakker met oogcontact bij stemgeluid (<10 seconde).
  • Matige sedatie: beweging of ogen open bij stemgeluid (geen oogcontact).
  • Diepe sedatie: geen reactie op stemgeluid, wel beweging en ogen open bij lichamelijke prikkeling.
  • Niet wekbaar: geen reactie op stemgeluid of lichamelijke prikkeling.
