Classificatie voor albuminurie als maat voor nierschade
Albuminurie is een belangrijke maat voor vaststellen van de mate van nierschade. Het onderhavige algoritme ondersteunt de medicus bij het op juiste wijze classificeren van de mata van albuminurie. 

NB: Voor het vaststellen van albuminurie zijn albumineteststroken onvoldoende nauwkeurig. Vaststelling van (micro-)albuminurie dient plaats te vinden in het laboratorium door bepaling van de albumineconcentratie of de albumine/creatinine-ratio (ACR).

Albuminurie: Van micro-albuminurie wordt gesproken bij:

  • een verlies in de urine van 2,5 tot 25 mg albumine/mmol kreatinine bij mannen en 3,5 tot 35 mg albumine/mmol kreatinine bij vrouwen in een willekeurige urineportie, of
  • 30 tot 300 mg albumine/dag in een 24-uurs urineverzameling, dan wel
  • 20 tot 200 mg/l in een willekeurige portie urine.
Van macro-albuminurie wordt gesproken bij:
  • een albumineverlies van meer dan 25 mg albumine/mmol kreatinine bij mannen en meer dan 35 mg albumine/mmol kreatinine bij vrouwen in een willekeurige urineportie, of
  • 300 mg albumine/dag in een 24-uurs urineverzameling, dan wel
  • 200 mg/l in een willekeurige portie urine.
Bij macro-albuminurie > 300 mg/dag is er door verlies van ook andere eiwitten dan albumine, ten minste een totale eiwituitscheiding van > 500 mg/dag. We spreken dan van proteïnurie.


Er bestaat een goede correlatie tussen de albumineconcentratie in één portie urine en de totale uitscheiding in de 24-uursurine (correlatiecoëfficiënt: 0,86 tot 0,90) [Bakker 2004]. Een albumineconcentratie in de urine van > 20 mg/l heeft een sensitiviteit van 82 tot 97% en een specificiteit van 74 tot 97% om een albumine-uitscheiding van > 30 mg in de 24-uursurine te voorspellen.

Een berekening van de albumineconcentratie naar de creatinine-uitscheiding in de urine (de albumine/creatinine-ratio) resulteert in een iets nauwkeuriger voorspelling van patiënten met een verhoogde albumine-uitscheiding. Deze ratio corrigeert voor concentratie- en verdunningseffecten van de urine. Een albumine/creatinine-ratio > 3,5 mg albumine/mmol creatinine heeft een sensitiviteit van 88 tot 100% en een specificiteit van 95 tot 99%.
